The Sunflower Advantage
Sunflower Adult Day Health Care Center (ADHC ) is a community-based adult services (CBAS) program that develops and delivers a comprehensive Sunflower Care Plan tailored to each individual and their unique physical, emotional, mental, and social needs.
Our team of over 25 members includes licensed nurses, social workers, occupational therapists, physical therapists, speech pathologists, and dieticians who meet you and your loved one where you’re at to guide and walk alongside you starting from visitation through becoming a participant in our lively community.
We are the only nonprofit adult day health care center in the San Gabriel Valley that provides the right care to empower older adults and those with chronic medical, cognitive, or behavioral conditions to continue living at home – not institutional care.
